Your Office Visit

At Ophthalmic Surgeons & Consultants of Ohio, Inc. we realize that a visit to the doctor can be a stressful experience, especially if you have a pain that won’t subside. In order to expedite your office visit, please review the following steps for simple hints and guidelines.

Before Your Office Visit

  • If you are a new patient, please complete the New Patient Packet that we will send to you prior to your visit. You can also download/print the packet from our Forms page in the Patient Information section. Please bring these forms with you – do not mail them back to us.
  • Bring an up-to-date list of all medications you are currently taking, including over-the-counter drugs, prescriptions, vitamins, and herbal products. This should include dosage, route, and frequency.
  • Bring a list of any known allergies and their effects – especially any concerning medication.
  • Know your personal and family medical history, including serious illnesses, surgeries, etc.
  • Jot down your symptoms or any concerns you have regarding your vision, including when your symptoms first appeared and anything that makes them better or worse.
  • Bring the glasses that provide you with your best vision, whether or not you wear them, including any over-the-counter glasses that you use.
  • Have the current contact information, including address, phone, and fax numbers of your referring physician, your primary care physician, and your preferred pharmacy.
  • Make sure you have an up-to-date photo ID and all current health insurance cards, so our front desk staff can update your file.
  • Check with your insurance company to ensure we are providers under your plan and what you should bring in respect to any co-pays or deductibles due at the time of your appointment.

During Your Office Visit

  • Review the items listed above with your doctor.
  • If you need a refill on any of your eye medications/drops, please let the doctor or technician know during your exam.
  • Comprehensive eye exams typically include dilating your pupils, which allows the doctor to see into the back of the eye. These drops can cause temporary light sensitivity and blurred vision, which may impact driving. You may wish to have a driver and bring sunglasses to your appointment.
  • Honoring your appointment time is very important to us and we plan ahead to ensure disruptions to schedules are kept to a minimum. However, since our physicians are subspecialists, they deal with the most complex diseases and procedures. This means that even during routine appointments, issues are often discovered during the exam, which requires immediate treatment or testing. On we may have to delay or reschedule your appointment time due to emergencies, urgent medical consultations, or surgical situations. We will make every effort to expedite your appointment or reschedule it quickly for your convenience.
  • Make sure you have a clear understanding of your role in treating your condition.
  • Ask your doctor questions, including what tests may be necessary, if drops or treatment are needed, and what he/she thinks the diagnosis/issue may be. Our doctors are here to provide you with the best quality care and your participation is crucial to the process.
  • Ask if a follow-up appointment with your doctor is required, or if you should be referred to another physician or specialist. Our technicians and front desk staff can help you with follow-up appointments.

After Your Office Visit

  • Follow any instructions the doctor gives you, including using all necessary drops.
  • If you do not understand the instructions, please do not hesitate to call our office at 614-221-7464 for clarification.
  • If any symptoms worsen or if you experience any side effects from any prescribed medicine/drops, contact our office immediately. For any urgent matters or life-threatening emergencies, call 911.
  • If you are scheduled for a follow-up appointment, mark your calendar and make sure you keep your appointment. If you are unable to keep your appointment, please call us as soon as possible, preferably over 24 hours in advance, so we can reschedule your visit. Repeatedly cancelling or failing to notify us to reschedule can jeopardize your relationship in our practice.
